Enrollment Details

• Enrollment fee of $100 for individuals, $150 for families (waived for children with adult membership.)*
• Subscription fee waived for each child after the 5th membership within the immediate family, residing in the same residence.
• Child – Dependent discount available for those living at home or with a valid student I.D. college student discount up to age 26 on a case-by-case basis.
• Subscription can be terminated within 24 hours’ notice by the patient.
• Automatic payment options: credit card charge or bank draft. No billing statements.
• Billing processed monthly, quarterly, or annually, with a 10% subscription discount for annual payments.
